ABSTRACT

Objective To evaluate the correlation between cerebral blood flow perfusion and memory impairment in patients with severe stenosis of vertebral basilar artery (VBA).Methods 62 cases of patients with VBA stenosis diagnosed by digital subtraction angiography(DSA) in Beijing Tiantan Hospital from September 2016 to March 2017 were enrolled.Mental State Examination (MMSE),Clinical Memory Scale (CMS) test and CT perfusion(CTP) was performed.All patients were divided into memory normal group(n=24,including 1 excellent case,6 above normal cases,and 14 normal cases) and memory impairment group(n =38,including 18 below normal cases,12 periphery cases,8 impaired cases) according to CMS.The ratios of side-to-side period were compared between bilateral mesial temporal lobe and anterior circulation area.The relative time to peak (rTTP),relative mean transit time(rMTY),relative cerebral blood flow(rCBF) and relative cerebral blood volume (rCBV) were calculate.Results The incidence of CTP decompensation in the medial temporal lobe was higher than that in the patients with memory impairment(P<0.05).The difference of rTTP and rMTT value between the two groups in the bilateral medial temporal lobes was statistically significant (rTFP:(1.131 ±0.037),(1.437±0.139),t=10.520,P< 0.05);rMTT:(1.081 ±0.059),(1.281 ±0.174),t=5.423,P<0.05).Conclusion The patients with VBA severe stenosis are more likely to get memory impairment due to cerebral hypoperfusion.

ABSTRACT

Objective To investigate the effect of stellate ganglion block(SGB)on heart mte variability (HRV)during thyroidectomy performed under cervical plexus block.Methods Forty-five ASA Ⅰ or Ⅱ pafients of both sexy8(8 males,37 females) aged 20-60 yr,weighing 40-70 kg undergoing elective thyroidectomy under cervical plexus block were randomly divided into 3 groups(n=15 each):group Ⅰ control,group Ⅱ left SGB and group Ⅲ right SGB.The parameters of HRV included low frequency(LF),high frequency(HF),LFnu,HFnu and LF/HF ratio.SBP,DBP,HB and HRV parameters were recorded before anesthesia(To,baseline),immediately(T1) and at 5 and 10 rain after cervical plexus block(T2,T3),at the beginning and end of operation (T4,T5).Results SBP and DBP were significantly increased at different time points after cervical plexus block in all 3 groups.Compared with the baseline at T0,LFnu and LF/HF ratio were significantly increased while HR,HF and HFnu decreased during operation in group Ⅰ,LF,HF and HFnu were significantly decreased while HR increased during operation in group Ⅲ(P<0.05),but no significant change in HRV parameters was found during operation in group Ⅱ(P>0.05).Compared with group Ⅰ,HR and LFnu were significantly decreased while HF and HFnu increased in group Ⅱ(P<0.05),but no significant change was found in the indexes mentioned abovein group Ⅲ (P>0.05).Conclusion SGB can adjust cervical plexus block-induced sympathetic excitability and maintain the balance between sympathetic and parasympathetic tension.Left SGB outweighs right SGB.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the clinical characteristics of radiation encephalopathy in nasopharyngeal carcinoma (NPC) patients of mainland China.</p><p><b>METHODS</b>Based on the principle of systematic review, the search for literature in computerized Chinese Biology & Medicine database, 1979 to June 2001 (CBM) was performed, with the papers appraised according to conformation criteria, the methodological quality of the studies analyzed, and clinical data of patients processed using the statistical model.</p><p><b>RESULTS</b>553 cases accumulated from 18 identified articles submitted by 16 hospitals in China were analyzed. The mean incidence of encephalopathy diagnosed by CT or MRI was 1.9% (0.4% - 2.6%). The disease focus was located in the temporal lobe (80%) and in the brain stem (17%). The mean dose received in the first course of radiotherapy to the nasopharynx or skull base was 72.9 Gy, and the latent period of this disease was 3.6 years.</p><p><b>CONCLUSION</b>The probability of encephalopathy after radiation for nasopharyngeal carcinoma in mainland China is reported in this review. Multi-center case-control or Cohort studies are required in the future.</p>
